Changing an Address on a Current License

FOR CORPORATIONS:

1.         A signed letter on corporate letterhead stating new address and phone number signed by an officer of the corporation.

2.         A new certificate of liability insurance with the new address and Consumer Affairs as the certificate holder

3.         Proof of your new location must be submitted with all address changes.  For acceptable proof

of your new location, a copy of the business phone bill showing the land-line business phone number must be submitted. Cell phones and toll free numbers are not permitted for this requirement. Home phones are acceptable as business numbers if you are doing business from your home.

 

FOR TRADE NAMES AND PARTNERSHIPS:

1.         You must amend your address and phone number with Nassau County Clerk (571-2270) and submit a copy of the filed amendment.

2.         A new certificate of liability insurance with the new address and Consumer Affairs as the certificate holder.

3.         Proof of your new location must be submitted with all address changes.  For acceptable proof of your new location, a copy of the business phone bill showing the land-line business phone number must be submitted. Cell phones and toll free numbers are not permitted for this requirement. Home phones are acceptable as business numbers if you are doing business from your home.

If a license with the new address is desired, the old license must be returned and a payment of $50 by certified check or money order must be submitted.
